FAQ

What do you call this membership-style of practicing medicine?

It is commonly referred to as Direct Primary Care (DPC) and is becoming increasingly popular across the country as both patients and doctors are discovering its advantages. It is an alternative to the fee-for-service model that most people currently use.

How does Direct Primary Care work?

The Direct Primary Care model covers primary care services including clinical work, consultative services, care coordination, and comprehensive care management. The DPC model also allows providers to procure wholesale and discounted lab work and prescriptions. Patients pay a monthly fee (i.e., a retainer) for all these services. Each DPC doctor is able to tailor their practice to their patients’ needs, such as offering house calls, telemedicine, email consults, and direct access to their physician as well as many other benefits.

Is Direct Primary Care the same as Concierge Medicine?

While the 2 models are similar, they’re not quite the same. Both provide fantastic care for patients but Direct Primary Care is typically less expensive and doesn’t involve insurance companies in any way.  See this article for more information about the Concierge and Direct Primary Care.

Do you take insurance?  What about Medicare?

I can see anyone regardless of insurance coverage.

I do not bill your insurance or Medicare but you can still use them for imaging, labs, medications, and specialist referrals.

What if I don’t have insurance?

Anyone can sign up regardless of insurance coverage.

Why pay to join your clinic when primary care is covered with my insurance?

Family physicians can provide comprehensive care to 80 – 90 percent of people at most times in their lives. Most insurances charge a co-pay, deductibles and give partial reimbursement so you end up paying the balance. This limits the amount of time people get to see their doctor, which leads to rushed office visits with many health concerns going unaddressed due to a lack of time. In this new model patients have 30-60 minute visits for an unlimited number of visits per year. There are no hidden costs as you pay the same amount each month. There are fewer hospitals stays, referrals and unnecessary imaging and labs, in effect saving you healthcare dollars.

How can I join?

Simply fill out the form on the Contact page and Dr. Rizvi will be in touch!

Will you be hard to reach?

I am limiting my practice size so we will have plenty of time to address all your healthcare needs. Since I can be reached directly via cell phone, email or video chat, I will be much easier to reach than most physicians you’ve seen at big-box medical clinics.

How many visits do I get per year?

There are no limits on the number of visits. Ideally, I would like to see you at least 3-4 times per year to ensure proper health care needs are met and you are on track to meet your wellness plan goals. There are no per-visit fees or copays!

What happens if I need to see a specialist doctor or need to have surgery?

My practice covers the primary care services as stated in the patient agreement.

If you need to see a specialist, I will refer you to one and provide them with all needed information; I will also coordinate any follow-up care you may need.

You can still use your insurance for visits to outside doctors and specialists (contact your provider to confirm coverage).

Will you be around if I need you in an emergency or for a hospital stay?

In case of an emergency you should call 9-1-1 immediately.

If you are admitted to the hospital, I would like to hear about it as soon as possible so I can ensure you receive quality care and follow-up treatment

What are the fees if I miss or cancel an appointment?

All appointments should be cancelled at least 24 hours in advance to ensure access for all patients.

A $20 late-cancellation fee will be charged for any appointments cancelled with less than 24 hours notice, unless there is an emergency or the patient is too ill to attend the appointment. Three unexplained no-shows in the span of one year will result in termination of Membership.

Can I see you for just one office visit?

Sorry, not at this time.

Do you take care of chronic pain patients?

Due to the changes in federal laws and monitoring requirements for narcotic use I will not be able to maintain long-term care/medications for chronic pain and I will prescribe narcotic medications only on a limited and short-term basis.

For long-term care, a referral will be made to a chronic pain specialist.
